Application Process

1.  Fill out scholarship application completely

2.  Personal Statement

Please include a Brief Essay with your application (to be attached to your email in morcosoft word format).  Please describe your reasons for wanting to engage in this course work or practicum and how it is relevant to your career and/or field.  How will this coursework or practicum help you as a professional.

3. Letter of Recommendation

Please include a Letter of Recommendation from one of your professors or employer, stating why you would be a good candidate for this scholarship.  Please email your recommendation to scholarships@telamonins.com 

4. Signature

Digitally Sign the Application at the bottom of the form by selecting "yes" or "no" based on the following statement;  I hereby declare that the information provided on this application is accurate and complete, and that I am the person whose name appears on this application.
